Individual living processes are complex. To simplify this process, registered provider My Supports created WayHome to help your clients find a Host or Housemate.
Finding a Host or Housemate is part of the bigger picture in Individualized Living and the involvement of you as the coordinator in this process

would be highly appreciated. You know your client and we would like to work hand in hand to create the best outcome for them. There are 5 steps to get an Individualized Living Option in place.

Once a connection is made and both parties want to start a living arrangement, we will start the Individualized Living process through NDIS registered provider My Supports.
In the service proposal we gather all the information regarding what services and supports are going to be included in the quote. Think about help in the house.

Time for the keys! Your Neighbourhood Team Leader will be in touch with you monthly to make sure both parties are happy and comfortable in their new arrangement.
WayHome is a tool created by registered provider My Supports and to get the best out of your ILO, the first 12 months of all arrangements will be implemented and managed by My Supports.
